Contact, Application & Payments

To Submit an Application and Payment:

Please download an Application Form by clicking on the hyperlink text below. A full description of the application process and options for the $150 USD application fee (online by credit card using secure PayPal or by check) are provided on the Application Form itself. If you are making an online credit card payment, you will click on the appropriate "Buy Now" or "PayNow" button below to go to the secure PayPal page and provide the appropriate information. An email confirmation will be sent to you should you choose to pay online. We suggest online payment to expedite your application. Online payment is accepted for most major credit cards for U.S. residents.

Non-U.S. residents can also remit online using a PayPal account funded in USD OR must pay by check in USD drawn on a bank with a U.S. correspondent bank. If remitting by check, it is recommended that non-U.S. residents consider making a single payment to cover both the Application Fee and Study Guide/Written Examinations Fee to save time and a second mailing.

If an application were not accepted or withdrawn by an applicant, all fees would be returned except a $50 USD processing charge.

	$500 USD for One-time Payment for Study Guide / Written Examinations
	$750 USD for Oral Examination
	$50 USD Additional for USPS Express International Mail
	$95 USD for Annual List Serve Membership Fee
	$30 USD for Reprinting of Additional Certificates or for Replacement Study Guide

Members and Diplomates:

To facilitate communication about current events, the Board urges all Members and Diplomates to contact the Board to confirm their current addresses and then to notify the Board whenever they move.
